Your company name, contact name, email and phone are included in the notification so we can reach you.
No password is required. Anyone with access to the delivered workbook can open it. Add password protection if you want an additional lock on the file.
At least 12 characters. A few words are easiest to read over the phone. We never see this password unless you give it to us.
Your entries stay in this page’s memory until you send them. Refreshing or closing this page clears them. Do not include Social Security numbers or medical diagnoses. A backup is retained in our private website administration for up to seven days, then scheduled for deletion. We use this information only to prepare your quotes. We do not sell it. Sending a census does not create coverage or bind any policy.
